Criteria – Colya Kaminiarz Professional Achievement and Education Award

Objective
To recognize a CSRT member who has made a significant contribution to the education or professional practice in respiratory therapy.

Rationale
This award is designed to honour Colya Kaminiarz for his contributions to the profession and professional association. The recipient should be a member of the CSRT who is recognized for his leadership skills and who has made a significant contribution to the profession and/or in the field of education at the national level.

Eligibility
Nominees should be current registered or non‐practising member of the CSRT in good standing. There is no restriction as to the number of years the nominee has been a member of the CSRT.

Nomination and selection
Nominations will be accepted from CSRT members  Nominations should be received by November 15, 2009. The nominee for the following year will be announced at the President’s banquet.

Judging criteria

• Nominee is a member in good standing of the CSRT
• Nominee is recognized as a leader in the profession and has made significant contributions to the practice environment or education process at the national level.
• Nominee has influenced others toward higher levels of achievement.

Prize

• Complimentary registration to conference and ticket to the President’s banquet
• Travel and accommodation for the conference
• Plaque presented at the President’s banquet
