Note: Please ensure all of your team members have already registered for the conference. All three team members must be registered for the conference at the time this form is submitted. Registrations where any team member has not registered for the conference will be returned.

Eight teams will be selected on a first come-first served basis. Two additional teams will be selected and placed on a wait list.

The format of the game show is a Family Feud style game. Team mates will require reliable internet access with camera and microphone for the competition.

Competitors on all teams (wait list team included) must be available from 1630-1810 EDT on May 5, 2021 to participate. Additional communications will be sent to teams accepted for the competition.

The CSRT will be recording sessions during this virtual event. The CSRT may also take screenshots throughout the conference. Videos and screenshots may be used for promotional purposes.
